What can I do if my Replacement Head breaks off in my Source handle?

Issue

I was brushing my teeth and the head broke of in the handle.

Please be sure to include your name, address, phone number, and what bristle and handle type you prefer. We will also send you a postage paid envelope to return the brush to RADIUS for testing.
